Job summary

Dr Mittal & Partners is seeking a Clinical Pharmacist to join Reading Holybrook PCN, delivering LTC management, medicines optimisation and prescribing safety within a supportive, learning-oriented primary care environment. The role involves collaboration with GPs, pharmacists and technicians and opportunities to pursue independent prescribing and specialist interests.

You will work across LTC reviews, med management in CVD/diabetes, and contribute to QOF and quality improvement, with mentorship

Qualifications

This is an exciting opportunity to join and help develop our expanding clinical pharmacy team. Reading Holybrook PCN is in the heart of Reading, serving over 37,000 patients at 3 practices. We are seeking an enthusiastic Clinical Pharmacist who is keen to develop their skills in primary care as part of our pharmacy team. This role offers the opportunity to work closely with Senior Pharmacists, GPs and pharmacy technicians while developing expertise in meds optimisation, LTC management and prescribing safety.

As an accredited learning practice with over 5 years experience in developing AHPs, we are committed to upskilling our workforce. We offer regular clinical supervision, mentorship and protected learning. You will be actively supported to develop specialist clinical interests and gain experience across a broad range of clinical areas, including: CVD, diabetes, respiratory disease, frailty and meds optimisation. If required with suitable experience, you will be supported to work towards an independent prescribing qualification.

Main duties of the job

Long-Term Condition Clinical Lead: Management of a designated LTC through clinical reviews; lead medicines optimisation initiatives; support QOF and quality improvement priorities; promote best practice and provide specialist advice.

Clinical Care: Support management of LTC including hypertension, osteoporosis, rheumatoid arthritis, CVD, diabetes and CKD. Prescribe safely as an Independent Prescriber within competence. Participate in PCN/pharmacist duty days, urgent reviews and medication queries. Provide expert medicines advice to GPs and the MDT, review blood results with GP trainer support, and support seasonal minor illness, acute care and vaccination services.

Medicines Optimisation: Conduct med reviews, inc care home, palliative and frail patients. Review high‑risk medicines and participate in prescribing, safety and CQC audits. Ensure prescribing complies with shared care agreements and the local formulary. Complete meds reconciliation following clinical correspondence.

Leadership & Service Development: Strategic development of PCN pharmacy services, workforce planning and service evaluation. Contribute to clinical protocols, SOPs, research and quality improvement. Support investigation of significant events and medication incidents.

Education & Supervision: Mentor junior colleagues, contribute to teaching, and support trainee/foundation pharmacists, including future pharmacists.
