Job summary

Reading Holybrook Primary Care Network (PCN) is seeking an enthusiastic Clinical Pharmacist to join our pharmacy team in primary care. • Review high-risk medicines and monitoring requirements.
• Manage complex medication reconciliation post hospital discharge.

Job summary

We are seeking an enthusiastic Clinical Pharmacist who is keen to develop their skills in primary care as part of our pharmacy team. This role offers the opportunity to work closely with Senior pharmacists, GPs and pharmacy technicians while developing expertise in medicines optimisation, long-term condition management and prescribing safety.

As an accredited learning environment with over 5 years experience in developing allied health professionals, we are committed to upskilling our workforce. You will be actively supported to develop specialist clinical interests and gain experience across a broad range of clinical areas, including: cardiovascular disease, diabetes, respiratory disease, frailty and medicines optimisation. Main duties of the job

As a Clinical Pharmacist, you will work with the pharmacy team to support medicines optimisation across the PCN. You will undertake structured medication reviews, support patients with long-term conditions, participate in prescribing safety initiatives and provide expert medicines advice to patients and colleagues. You will work closely with Senior Clinical Pharmacists to develop skills in long-term condition management, quality improvement and clinical leadership. You will also have the support of pharmacy technicians and a dedicated pharmacy administrator. If you have not already completed the CPPE PCEP course, you will be enrolled onto this shortly after joining us.

Qualifications

Essential

• GPhC registered pharmacist

• Experience supporting patients with long term conditions.


• Interest in developing a specialist clinical area.
