Job summary

Belper PCN is seeking a part-time Clinical Pharmacist to join our supportive primary care team in Belper. The role focuses on medicines management, reviews, and safe prescribing in line with NHS guidelines.

You will work with practice teams, receive guidance from the Lead Pharmacist, and have access to ongoing development opportunities as we strive to improve patient care through collaborative practice.

Qualifications

• MPharm degree and GPhC registration are required.

Responsibilities

We are seeking to recruit a Part-time Pharmacist required to work 16-18 hours per week including Fridays.

Main duties of the job

Primary care is a fast moving and adaptable environment where priorities can change to match the changing needs of our patients. The focus of the role is, to work with the practice health care teams to rationalise and manage medicines, encourage safe prescribing, and optimise medicine review processes. Whilst taking into consideration local ICB and national NHS targets and objectives.

The PCN has a history of adopting new approaches in community care and already has an existing team of pharmacists and advanced care practitioners who have delivered real change and improvements to patient care.

Job responsibilities

• Patient facing long-term condition Clinics - be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities.
• Clinical medication reviews - Undertake clinical medication reviews with patients and produce recommendations for safer prescribing and monitoring. Supporting patients to practice self-care and reduce risks of long-term harm related to medications.
• Care Home Medication Reviews - Undertake clinical medication reviews to produce recommendations for the Lead ACP or GP on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration.
• Patient facing Domiciliary Clinical Medication Reviews -Where needed undertake clinical medication reviews with patients and produce recommendations for prescribing and monitoring. Attend and refer patients to multidisciplinary case conferences.
• Risk stratification - Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
• Unplanned hospital admissions - Review the use of medicines most associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.
• Patient medicines support - Provide face-to-face and telephone clinics for those with questions, queries and concerns about their medicines in the practice.
• Medicine information to practice staff and patients - Answer relevant medicine-related enquiries from GPs, other network staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.
• Signposting - Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.
• Repeat prescribing Support the implementation of a repeat prescribing policy within each PCN practice. Help oversee the repeat prescribing reauthorisation process to ensure medicines reaching review dates are highlighted and flagging up those needing a review. Ensure patients have appropriate monitoring tests in place when required.
• Service development - Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
• Information management - Analyse, interpret and present medicines data to highlight issues and risks to support decision making.
• Medicines quality improvement - Undertake clinical audits of prescribing in areas directed by the PCN lead pharmacist, feedback the results and implement changes in conjunction with the relevant practice team.
• Medicines safety - Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
• Implementation of local and national guidelines and formulary recommendations - Monitor practice prescribing against the local health economys RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs). Assist practices in seeing and maintaining a practice formulary that is hosted on each practices computer system. Auditing practices compliance against NICE technology assessment guidance. Provide newsletters or bulletins on important prescribing messages.
• Education and Training - Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
• Care Quality Commission - Work with the general practice teams to ensure the practices are compliant with CQC standards where medicines are involved.
• Clinical supervision actively participate in monthly group clinical supervision and quarterly one-to-one peer support sessions to share learning experiences and support team development.
• Continual Professional Development (CPD) ensure CPD requirements for GPhC registration are met and always maintained.
