Job summary

Walsall Healthcare NHS Trust is seeking a Clinical Sister / Charge Nurse / Clinical Practitioner to join its Care Navigation Centre. You will be a registered nurse or paramedic, with Band 5 practice experience, and will triage and refer patients to community and acute services.

Qualifications

• Must be a registered practitioner (nurse/paramedic) with evidence of post registration practice at Band 5.
• Experience of multi-disciplinary / multi-agency working is essential.
• Good IT skills and a pleasant telephone manner.

Responsibilities

• Triage and assess care needs and develop programmes of care within scope of professional practice.
• Demonstrate high level judgement in interpreting and performing patient assessments and care planning via telephone and face to face.
• Maintain accurate and up-to-date patient care records and uphold confidentiality.
• Collaborate with other professionals to promote multidisciplinary / multiagency working.
• Gain access to enhanced programmes of care through integrated care pathways.
