Job summary

Walsall Healthcare NHS Trust is inviting applications for a Clinical Sister / Charge Nurse / Clinical Practitioner within the Care Navigation Centre. This Band 6 post requires post-registration practice and multi-disciplinary collaboration to triage and refer patients to community services.

Qualifications

• Registered Nurse or Paramedic with post registration practice.
• Experience in primary/secondary care settings advantageous.
• Evidence of ongoing professional development.

Responsibilities

• Triage and assess care needs by telephone and in person.
• Coordinate multidisciplinary care plans and referrals.
• Maintain accurate patient records and uphold confidentiality.
• Promote multidisciplinary working with community and acute services.
• Ensure professional practice in line with Trust policies.
