Job summary

The role requires leadership of care planning processes and ensuring smooth discharges and risk assessment are in place. Ideal candidates will have NVQ 3 or equivalent in care for care homes, at least two years in a senior role with residents,

Qualifications

• Leadership experience in a senior resident-facing role.
• Strong communication skills, both verbal and written.
• Ability to lead staff and act responsibly in manager's absence.
• Commitment to person-centred, compassionate care.

Responsibilities

• Lead in the person-centred care planning process, monitor evaluations and audits.
• Oversee smooth discharge processes and keep residents' next of kin informed.
• Monitor resident well-being, assess care changes and update records accordingly.
• Ensure safe systems of work and risk assessments across tasks.
