Job summary

• Experience in care coordination and caseload management.
• Ability to provide professional leadership and guidance within a multidisciplinary team.

Responsibilities

• Manage a caseload of service users autonomously (15–20).
• Provide professional leadership and clinical guidance within the team.
• Contribute to service improvement and mentorship of staff.
• Collaborate with clinicians and external partners to promote MH Practitioner role.
• Foster networking and partnerships with internal and external stakeholders.
• Plan, implement and evaluate evidence-based care programmes for service users.
