Job summary

Leicestershire Partnership NHS Trust is seeking a Community Connector to join the Mental Health Planned Care Services in Leicester. The role involves working with adults and older adults in neighbourhood teams, linking service users with voluntary and community organisations, and promoting wellbeing and independence.

You will operate within a multidisciplinary team, provide support to design care plans, and contribute to a wider transformation project aimed at delivering closer-to-home services.

Responsibilities

• Assist in providing a professional, efficient, and seamless service; first point of contact for individuals referred into the neighbourhood team.
• Deliver high-quality, individual sessions to patients, providing advice and information to promote healthier lifestyles and recovery.
• Assess risk and needs and develop intervention plans to address needs through brief interventions.
• Link and support service users to voluntary and community sector organisations; form links with local external agencies.
• Demonstrate the Trust’s values and undertake holistic assessments to understand care needs and plan relevant supports.
