North London NHS Foundation Trust is seeking a Senior Care Coordinator within the Learning Disability and/or Autism Spectrum Condition Keyworker Service for Barnet, Enfield and Haringey.
You will coordinate the Dynamic Support Register for children with learning disabilities and/or autism with mental health needs, chairing CETRs/LAEPs and supporting community-based care planning to prevent unnecessary admissions.
You will be a Senior Care Coordinator within the Learning Disability and/or Autism Spectrum Condition Keyworker Service for Barnet, Enfield and Haringey: providing coordination of the Dynamic Support Register for children with learning disabilities and/or autism with mental health needs and/or presenting with behaviours of concern.
The LD/ASC Senior Care Coordinator provides intensive and highly skilled administration support to the systems around children and young people who are part of the LD & ASC cohort. This includes chairing of meetings that identify strategies and provision that prevent breakdown of the child’s current placement and a deterioration in their presentation. The aim is to keep children, young people in the community, to prevent avoidable admissions A & E, Tier 4 inpatient mental health services and 52-week residential placements, and to support young people in this cohort, to be discharged from Tier 4 services and be supported back into the community.
The service is working across Barnet, Enfield, Haringey, Camden and Islington, with work allocated between two Care Coordinators. The two care Coordinators will come together to ensure both locality teams work together to develop shared processes and protocols as well as reporting systems.
