Job summary

North London NHS Foundation Trust is seeking a Senior Care Coordinator within the Learning Disability and/or Autism Spectrum Condition Keyworker Service for Barnet, Enfield and Haringey.

You will coordinate the Dynamic Support Register for children with learning disabilities and/or autism with mental health needs, chairing CETRs/LAEPs and supporting community-based care planning to prevent unnecessary admissions.

Responsibilities

  • Manage scheduling and chair/support CETRs/LAEPs for children and young people across Barnet, Enfield Haringey, Camden and Islington identified by LD/ASC service to enable community-based care planning.
  • Ensure NWL ICB and NHSE Specialised Commissioning deliver good practice and KPIs, including LAEP CETRs when capacity allows.
  • Review CETR/LAEP recommendations and liaise with commissioners to highlight barriers/trends for future commissioning.
  • Attend discharge planning meetings such as inpatient MDTs, CETRs, CPA in CAMHS inpatient or residential settings.
  • Collaborate with Commissioners, CAMHS, Children’s Disability Teams, social care and Adult LD/MH teams to develop cross-NCL transition planning.

  • Job Summary

You will be a Senior Care Coordinator within the Learning Disability and/or Autism Spectrum Condition Keyworker Service for Barnet, Enfield and Haringey: providing coordination of the Dynamic Support Register for children with learning disabilities and/or autism with mental health needs and/or presenting with behaviours of concern.

The LD/ASC Senior Care Coordinator provides intensive and highly skilled administration support to the systems around children and young people who are part of the LD & ASC cohort. This includes chairing of meetings that identify strategies and provision that prevent breakdown of the child’s current placement and a deterioration in their presentation. The aim is to keep children, young people in the community, to prevent avoidable admissions A & E, Tier 4 inpatient mental health services and 52-week residential placements, and to support young people in this cohort, to be discharged from Tier 4 services and be supported back into the community.

The service is working across Barnet, Enfield, Haringey, Camden and Islington, with work allocated between two Care Coordinators. The two care Coordinators will come together to ensure both locality teams work together to develop shared processes and protocols as well as reporting systems.

3.1 Main Duties and Responsibilities
  • Manage the scheduling and chairing/supporting of CETRs/LAEPs for children and young people across Barnet, Enfield Haringey, Camden and Islington, who have been identified as part of the LD and/or Autism Service and are at risk of admission, in order to support delivery of a multi-agency approach to care planning which enables children and young people to be effectively supported in the community.
  • Ensuring that NWL ICB and NHSE Specialised Commissioning are delivering on good practice and key performance indicators such as holding LAEP meetings, three-monthly CETRs where capacity allows.
  • Reviewing CETR/LAEP recommendations and liaising with commissioners to highlight emerging barriers and trends to inform future commissioning and service delivery decisions.
  • Attending meetings which provide an opportunity to discuss discharge planning such as inpatient MDTs, CETRs, CPA, taking place either at CAMHS inpatient settings or residential settings.
  • Working with Commissioners, CAMHS, Children’s Disability Teams, social care and Adult LD and MH teams to develop arrangements that support effective transitions planning across NCL.
