Responsibilities

• Monitor residents' wellbeing and update care plans as needs change.
• Be involved in person-centred care planning and monthly evaluations.
• Assist residents with medication as required.
• Undertake staff supervisions and support professional development.
• Act as a key contact for residents, relatives and health professionals.
