Responsibilities

• Monitor residents' wellbeing and update care plans as needs change.
• Participate in person-centred care planning with monthly evaluations.
• Support residents with their medication requirements.
• Undertake staff supervisions and support staff development.
• Act as key contact with residents, relatives and health care professionals.
