Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust seeks an experienced Band 6 Senior Mental Health Practitioner to join the CAMHS SPA team. You will triage referrals, review information, assess risk, and direct young people to the most suitable support pathway.

You will work with children, young people and families, providing crisis guidance, signposting, and collaborative care planning while ensuring safe information sharing and strong governance across the service.

Qualifications

• Registered professionals (NMC/SWE/HCPC) eligible to apply.
• Experience in CAMHS or child/adolescent services preferred.
• Strong communication and safeguarding skills.

Responsibilities

• Undertake clinical triage and assessment of referrals to CAMHS SPA.
• Gather information on young people’s mental health and family context.
• Complete risk assessments and develop safety plans; escalate high‑risk cases.
• Provide advice, signposting and crisis intervention within scope of the role.
• Determine appropriate pathways of care (CAMHS, early help, community services).
• Signpost to families, carers, professionals and partner agencies.
• Collaborate with multidisciplinary colleagues and partners for coordinated care.
• Maintain accurate clinical records and ensure information sharing.
• Contribute to demand management, waiting lists and service improvement.
• Identify learning from incidents and contribute to service development.
