Job summary

NHS Forth Valley is seeking a Band 7 Advanced Practitioner to manage frail patients, perform CGA screening, and coordinate multidisciplinary care. The post involves leading medical reviews and working across health and social care boundaries.

The ideal candidate will hold an HEI-accredited prescribing qualification (V300) and have advanced practice credentials with evidence of CDP. PVG scheme is required.

Qualifications

• Essential: HEI accredited Independent Prescribing Qualification - V300.
• Essential: HEI accredited Clinical Examination & history taking course or equivalent.
• Essential: HEI accredited decision-making module or equivalent.
• Desirable: Post graduate master’s in Advanced Practice or equivalent.

Responsibilities

• Provide daily medical review of frail patients with MDTs.
• Lead and prioritise care using Comprehensive Geriatric Assessment.
• Communicate clearly with patients and carers about conditions and planning.
• Collaborate across organisational boundaries with health and social care teams.
