Job summary

HCRG Care Group in Swindon is seeking a Senior Advanced Practitioner (Band 8b) to lead expert clinical practice with leadership responsibilities across Bath and Swindon. The role bridges ACP and Consultant Practitioner capabilities while maintaining a strong clinical focus.

The post offers part-time or full-time flexible hours with a Mon-Fri 8am-4pm schedule and opportunities for continuing professional development within our Learning Enterprise. Interviews scheduled week of 23 November 2026.

Qualifications

• MSc Advanced Clinical Practice, or equivalent.
• Significant autonomous advanced practice experience.
• Leadership and service improvement experience.
• Current full driving license and use of a car during working hours.
• Experience of non-medical prescribing within frailty, community or long-term conditions setting.
• Teaching, mentorship or clinical supervision qualification (e.g. PGCert in Education or equivalent).
• Evidence of leading or contributing to audit, research or service improvement projects.
• Experience of working across multi-agency or multidisciplinary community settings.

Responsibilities

• Deliver highly autonomous, expert clinical practice for patients with complex frailty and long-term conditions.
• Lead complex clinical decision-making and multidisciplinary working.
• Support admission avoidance, rehabilitation and discharge planning.
• Provide professional leadership and visible expert practice within multidisciplinary teams.
• Lead quality improvement, service redesign and pathway development projects.
• Support governance, clinical effectiveness and implementation of evidence-based practice.
• Provide supervision, coaching and development for ACPs and trainee practitioners.
• Contribute to research, audit and innovation activity.
