Job summary

HCRG Care Group is seeking a Bank Advanced Clinical Practitioner to work across BSW localities, providing autonomous ACP services in Community Hospital inpatient settings. You will perform high-level assessments, coordinate investigations, and support discharge planning while leading a multidisciplinary team.

The role requires post‑registration experience at Band 7/8a level, an independent prescriber qualification, and a postgraduate nursing specialty.

Qualifications

• Registered Nurse with advanced practice registration.
• Band 7 or 8a level experience in community/acute nursing.
• Registered Independent Non Medical Prescriber.
• Leadership qualification or training.

Responsibilities

• Deliver autonomous Advanced Clinical Practitioner services in community inpatient settings.
• Conduct high-level assessments, diagnose, initiate treatments and coordinate investigations.
• Provide professional leadership to nurses and multidisciplinary teams.
• Support safe discharge and maintain information governance.
• Drive quality improvement through audits and learning.
