Job summary

NHS Jobs in Bath invites applications for a Senior Advanced Practitioner (Band 8b). The post delivers autonomous clinical practice for patients with complex frailty and long-term conditions, leading complex decision-making and multidisciplinary teams.

You will drive quality improvement, service redesign and pathways, provide supervision for ACPs, and contribute to research, audits and implementation of evidence-based practice.

Qualifications

• Registered healthcare professional with MSc Advanced Clinical Practice or equivalent
• Independent Prescriber qualification is required
• Significant autonomous advanced practice experience
• Leadership and service improvement experience
• Current full driving licence and use of a car during working hours

Responsibilities

• Deliver highly autonomous, expert clinical practice for patients with complex frailty and long-term conditions.
• Lead complex clinical decision-making and multidisciplinary working.
• Support admission avoidance, rehabilitation and discharge planning.
• Provide supervision, coaching and development for ACPs and trainee practitioners.
• Lead quality improvement, service redesign and pathway development projects
