Job summary
South West Yorkshire Partnership NHS Trust in Barnsley is seeking an experienced nurse to join the Urgent Community Response Service (UCR) as an Advanced Nurse Practitioner. You will deliver high level clinical assessments in patients' homes, develop care plans to prevent hospital admissions, and lead on clinical practice within the team.
The role requires Nurse Prescriber and APACS qualifications, a background in community nursing, and willingness to work across a seven-day pattern.
Qualifications
- Must have Nurse Prescriber course and APACS qualification.
- Experience delivering nursing care in community settings.
- Willingness to travel between sites and patient homes.
Responsibilities
- Provide 2-hour crisis response and advanced nursing assessment.
- Deliver care in patients' homes with comprehensive care plans.
- Support clinical leadership and supervision within the MDT.
- Contribute to service development and continuous improvement.
