Job summary

LOROS Hospice is looking for an experienced Advanced Clinical Practitioner to join the ICSPC Team, delivering expert palliative and end-of-life care in the community across Leicester, Leicestershire and Rutland. You will work autonomously with consultants and neighbourhood teams to assess, diagnose, prescribe, and manage care, aiming to keep patients at home where possible.

Qualifications

• Advanced clinical skills in palliative and end-of-life care, with ability to assess and manage complex conditions.
• Strong decision-making and autonomous practice with a person-centred approach.
• Experience in palliative, end-of-life, cancer or complex community care.

Responsibilities

• Undertake assessment, diagnosis, prescribing, care planning and clinical management.
• Provide visible clinical leadership, supervision and education across services.
• Support timely discharge and prevent avoidable hospital admissions.
