Job summary

HCRG Care Group in Swindon is seeking an Advanced Clinical Practitioner (Band 8a) to join the Integrated Medical Team at Swindon Intermediate Care Centre. You will deliver autonomous clinical care across community hospital services, handling complex and frailty-related conditions.

We offer part-time or full-time flexible working hours, with standard hours Monday–Friday; interviews are planned during the week of 23 November 2026.

Qualifications

• Registered healthcare professional.
• MSc Advanced Clinical Practice.
• Significant post-registration clinical experience.
• Full driving licence and car use during work hours.
• Experience in community hospital, rehabilitation or frailty services.
• Evidence of audit, quality improvement or service development activity.
• Experience mentoring junior staff or students.
• Membership of a relevant professional network or special interest group.

Responsibilities

• Deliver autonomous advanced clinical assessment, diagnosis and treatment.
• Manage patients with complex, frailty-related and deteriorating conditions.
• Coordinate patient care across organisational and professional boundaries.
• Support admission avoidance and timely discharge planning.
• Lead and contribute to clinical audits and service improvements.
• Promote evidence-based practice and continuous quality improvement.
