The Lambeth Living Well Centres serves a diverse population of over 1.1 million across the boroughs of Lambeth, Lewisham, Southwark and Croydon, as well as offering national services for patients who are resistant to treatment.
It provides treatment for 9,600 people, of whom 5,600 are on the Care Programme Approach, and its services include acute inpatient care, complex care, and community teams covering recovery and early intervention. It has 1,900 staff in 74 teams, and 270 acute inpatient beds, 44 PICU beds, 86 Rehabilitation beds and 300 Supported accommodation placements.
The Lambeth Living Well Centre has an annual budget of 128.5 million (health and social care). A robust Financial Strategy Plan has been developed to achieve financial balance over three years, including elimination of agency/bank costs, introduction of non-pay controls and elimination of overspill.
Care and Promoting Recovery. The development of the four care pathways is a major service transformation programme and the key strategy for maximising service efficiency and delivery of savings without adversely affecting patient care, including the consistent setting and monitoring of standards across the pathways.
1. Responsible for the day-to-day administration within the community service.
2. To be proficient with Microsoft office applications on behalf of the clinical team, including booking appointments with the consultants and the care co-ordinators.
3. Deal sensitively with patients, patient's relatives and clinical staff and ensure that urgent matters are drawn to the attention of relevant staff quickly and accurately.
4. Responsible for the recording, maintaining, updating and accuracy of clinical activity and patient information pertaining to the service. Ensure activity on the ePJS is accurate.
5. To process referrals accurately for the team.
6. To pull data accurate date from epjs and Deming as required for the team.
7. To be involved with the induction and liaising with digital services to get new staff to the team onto the system.
8. Maintain office systems, enabling the prompt retrieval of information.
9. To maintain and ensure high levels of data collection and data quality for the service, achieving 100% accuracy in specific identified areas in line with Trust targets.
10. To support and work in close liaison with multi-professional team.
11. To be involved in staff meetings and discussions as appropriate.
12. To have basic knowledge of digital services to assist colleagues with IT tasks.
13. To take minutes as required at various meetings and attend Teams morning handover meetings as required.
14. Support the service wide and local administrative cover arrangements.
15. To work on other sites as required.
16. To be responsible for booking interpreters, request for vouchers as required.
17. To provide Ad-hoc administrative and project support
18. Process e-procurements requests when required.
19. To undertake task deemed necessary to the grade and the needs of the service.
