West Northamptonshire Council is recruiting a SEND Practitioner to work with children aged 0-5 and their families. You will deliver Portage in homes and support children in nurseries, schools and Family Hubs, building skills and confidence for lasting progress.

You'll visit families in the community and collaborate with educational settings to enable inclusion, communication and independence, while supporting parents to continue strategies after your visit.

Qualifications

• Experience working with children and families in early years settings is desirable.
• Ability to build relationships with carers, educators and professionals.
• Strong communication and collaborative skills.

Responsabilités

• Support families in their homes with Portage home teaching and practical development activities.
• Visit nurseries, schools and with childminders to explain and share approaches for learning and inclusion.
• Help parents and carers apply strategies to support communication, independence and everyday skills.
• Contribute to Family Hubs and community activities to connect families with support and opportunities.
• Assist with assessments, planning and reviews while focusing on hands-on delivery and outcomes.
