Job summary

West Northamptonshire Council is seeking a SEND Practitioner to work with children aged 0-5 and their families. You will deliver Portage home teaching and support families in nurseries, schools and Family Hubs to promote development and inclusion.

You’ll build collaborative relationships with professionals, contribute to assessments and plans, and help children access opportunities to reach meaningful progress in everyday life.

Responsibilities

• Deliver Portage home teaching to families in a variety of settings
• Support children aged 0-5 in nurseries, schools and Family Hubs
• Build relationships with families and practitioners to support communication, learning and inclusion
• Assist with assessments, planning and reviews as part of hands-on support
• Contribute to safeguarding, welfare checks and early intervention strategies
