Sarcoma Advanced Nurse Practitioner, Band 8a

The ANP will be expected to play a lead role in the organization and running of the sarcoma diagnostic service. This entails triage of referrals, running the diagnostic clinic with the consultant staff, covering the resultant telephone follow-up clinic, and allied major and minor operating lists.
The unit has a substantial diagnostic service, via the Two Week Wait pathway, which has grown and evolved substantially in recent years. We have established diagnostic clinics with allied telephone follow-up and minor and operations lists. All new cases are reviewed at a weekly multidisciplinary meeting where management is discussed. Surgical management included soft tissue tumours of the limbs and limb girdle, intra- abdominal and trunk, and minimally invasive surgery where appropriate.
The ANP will undertake routine advanced physical assessment of surgical patients including history taking, clinical examination, arranging relevant investigations and prescribing relevant medications.
The ANP will ensure each patient has a plan of care within 24 hours of their admission.
Attending and presenting on Ward rounds – preparing Ward rounds / MDT/Theatre lists in conjunction with the theatre scheduling team. Review patient’s progress on a daily basis, recording any changes in their condition in the medical notes/annotate on EPR.
Main duties of the job
This post is aimed at developing new ways of working for senior clinical nurses as Advanced Nurse Practitioner (ANP). With mentor support, the sarcoma surgical ANP will integrate into the existing surgical teams to carry out their extended roles which will include:
- Routine ward management of the surgical patient, including practical procedures and the admission and clerking of surgical patients
- Contributing to outpatient clinics
- Liaising with the MDT team to help facilitate pathways within the confines of the cancer wait time targets as part of a patient tracking list (PTL) meeting.
- The ANP will be expected to play a lead role in the organization and running of the diagnostic service.
- They will act as a highly specialised role model and leader for all members of staff and play a major role in the continuous development of the services within the Trust and as a potential member of a range of trust quality/process improvement project groups.

Detailed job description and main responsibilities
Clinical
- The ANP will use the best available evidence, advanced specialist skills, knowledge and expertise to provide expert clinical care.
- They will integrate into the existing clinical structure and to support other professionals to manage the routine care of surgical in- patients by collaborating with other principle team members.
- The ANP will undertake routine advanced physical assessment of surgical patients including history taking, clinical examination, arranging relevant investigations and prescribing relevant medications. The ANP will ensure each patient has a plan of care within 24 hours of their admission.
- Attending and presenting on Ward rounds – preparing Ward rounds / MDT/Theatre lists in conjunction with the theatre scheduling team. Review patient’s progress on a daily basis, recording any changes in their condition in the medical notes/annotate on EPR. Verbally communicating changes in care to the patient and relevant ward nursing team.
- The ANP will undertake routine management of the surgical patient including regular monitoring of these patients, communicate with the ward staff where clinical concerns arise and be responsible for ensuring timely and safe discharge.
- Carry out clinical procedures, e.g. cannulation, blood sampling, arterial blood gas sampling, and other procedures relevant to their specialty following appropriate clinical sign off.
- The ANP will order investigations (e.g. x rays, CT scans, blood tests) and interpret results and escalate concerns and abnormalities as appropriate to the SpR/Consultant/Outreach teams.
- To provide support and advice to nursing, medical and allied health professional therapy staff in relation to the management of the surgical patient.
- To provide support and advice to patients and their carers relating to surgical procedures.
- Participates in the pre-admission services alongside the Pre-Admission Clinic Nurse where required, and take responsibility for organising, undertaking and interpreting investigations as appropriate to ensure patients are fit as possible for theatre
- To liaise with ward Sisters / Charge Nurse administrative staff, and other members of the MDT to optimise the smooth transition of surgical patients through the surgical pathway to discharge home. Liaise with surgical patient’s medical team and ensure that Consultant Medical team aware of any major change / problems regarding their patients.
- To ensure TTA medications are prescribed in a timely manner and discharge plans are provided and explained to patient and carers.
- The ANP will triage referrals, running the diagnostic clinic with the consultant staff, covering the resultant telephone follow-up clinic, and allied major and minor operating lists.
- Following protocol, they will ensure through good practice that patient care initiatives are improved and sustained and act as a resource for expert advice to patients, relatives/carers and staff.
- To lead the provision of care in accordance with the Trust Policies reflecting current trends and developments in practice and the NHS.
- To comply with and participate in the Trust’s framework for Clinical Governance within the Department area.
- Develop tools to improve the quality of the service and co-ordinate clinical services across the Department.
- Maintaining and updating Intermediate or Advanced Life Support qualification
- Ensure that documentation is maintained to the Trust and NMC standards
- To actively promote a healthy lifestyle for patients & staff appreciating national strategy requirements.
- Prescribe medication within set patient group protocols and current legislation. Develop patient group directives in the administration of patients medication for the use of other staff where appropriate
- Assist in the patient’s safe transition from hospital into the community with effective discharge planning. Liaising with other members of the multidisciplinary team to achieve safe and efficient discharge.
- Contribute to outpatient clinics, and running a robust nurse-led clinic service
- Attend weekly meeting with the MDT co-ordinators, performance team and CBU management to review patients on a cancer pathway and ensure patients are seen within the expected time frames set out by cancer wait time targets.
- To support the Sarcoma team with development of new diagnostic pathways and clinical triage models.
- To be involved with the management and triage of patients within the two-week rule pathway
