Responsibilities

• Make yourself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.
• Make autonomous clinical decisions in relation to presenting problems within the organisation.
• Assess healthcare needs of patients with undifferentiated problems and develop care plans in line with disease management protocols.
• Provide counselling and health education; admit/discharge patients and refer to other care providers as appropriate.
• Record clear, contemporaneous consultation notes; participate in audits and QOF activities; contribute to Read-Coding of patient data.
