Job summary

Ashfields Primary Care Centre in Sandbach, Cheshire, is seeking a Salaried GP to join our expanding clinical team on a permanent basis. The post focuses on proactive care home services and developing patient-centred care, with in-practice clinics and home visits.

You will contribute to safeguarding high-quality care across routine and urgent appointments and undertake administrative tasks such as prescribing and patient reporting.

Qualifications

• Must have GMC registration and be able to practice autonomously
• Experience delivering high-quality, proactive care in a GP setting
• Ability to manage complex elderly patients and create care plans including End of Life arrangements

Responsibilities

• Assess and manage patients presenting with a wide range of conditions in surgery and home visits.
• Practise autonomously within governance framework and Practice direction.
• Make timely referrals to secondary care and community services.
• Support shared decision-making with patients.
• Prescribe safely and cost-effectively.
• Maintain accurate clinical records using the Practice system.
• Participate in investigations of complaints and incidents.
• Engage in Performance Reviews and CPD planning.

An exciting opportunity has arisen for a motivated and forward-thinking 6 session Salaried GP to join our expanding clinical team at Ashfields Primary Care Centre on a permanent basis. This role offers the opportunity to shape and further develop our proactive care home service within a large, well-established, and high-performing practice. You would be joining a successful 28,000+ patient practice committed to quality improvement, innovation, and delivering excellent clinical care. We pride ourselves on fostering a supportive culture where clinicians are valued, their ideas are heard, and service development is actively encouraged., This is an excellent opportunity for a Salaried GP who is passionate about delivering high-quality, proactive care and who wishes to play an active role in shaping services within a supportive and progressive practice environment. The GP will have in practice clinics attending to both routine and urgent appointments. The successful candidate will also be responsible for administrative duties including signing prescriptions, completion of patient reports and filing patient results. Our in practice clinics consist of fourteen 15/12 minute appointments. Morning clinics run from 08.30am-12.30pm and afternoon clinics run from 15.00pm-18.30pm., We have:

• A supportive and well-established clinical team including Nurses, HCA's, Pharmacists and Pharmacy Technicians
• A dedicated pharmacist team providing chronic disease management, drug titration clinics, and post-discharge medication reviews
• First Contact Physiotherapists, Care Coordinators, and a comprehensive administrative team

Many of our GP Registrars have progressed into Salaried and Partnership roles within the practice. As an active member of our Primary Care Network, we maintain close working relationships with local care networks and community services, The Salaried GP will have both Clinical and Operational responsibilities as outlined below:

• To independently assess, examine, diagnose and manage patients presenting with a wide range of medical conditions, delivering evidence-based care in both surgery and home visit settings in accordance with current best practice guidelines.
• To practise autonomously whilst working within the clinical governance framework and strategic direction of the Practice.
• To make timely and appropriate referrals to secondary care and community services, working collaboratively with multidisciplinary teams and external agencies to ensure safe, coordinated, and patient-centred care.
• To support shared decision-making by offering patients informed choice and involving them actively in their care planning.
• To prescribe safely, appropriately, and cost-effectively in line with national guidance, local prescribing formularies, and Practice policies.
• To maintain accurate, systematic, and contemporaneous clinical records using the Practices electronic clinical system, adhering to agreed coding templates and data quality standards.
• To participate fully in the investigation and management of complaints and significant events within the designated clinical area, in line with Practice policies and clinical governance frameworks.
• To report all critical incidents, accidents, and near‑misses in a timely manner, and to actively engage in subsequent review processes, action planning, and monitoring to support continuous quality improvement and patient safety.
• To participate in Individual/Team Performance Review, attend mandatory training courses and complete an Annual Personal Learning Plan to identify individual training and development needs.

In addition to routine practice-based clinical sessions, this role offers the opportunity to take a lead or enhanced role in:

• Proactive care home ward rounds
• Comprehensive frailty assessments and personalised care planning
• Structured medication reviews in collaboration with our pharmacy team
• Advanced care planning and end‑of‑life care discussions
• Reducing avoidable hospital admissions through anticipatory care
• Developing strong working relationships with care home staff and community teams
  Full registration with GMC
