Job summary

Ashfields Primary Care Centre in Sandbach, Cheshire, is seeking a Salaried GP to join our expanding clinical team on a permanent basis. The post focuses on proactive care home services and developing patient-centred care, with in-practice clinics and home visits.

You will contribute to safeguarding high-quality care across routine and urgent appointments and undertake administrative tasks such as prescribing and patient reporting.

Qualifications

• Must have GMC registration and be able to practice autonomously
• Experience delivering high-quality, proactive care in a GP setting
• Ability to manage complex elderly patients and create care plans including End of Life arrangements

Responsibilities

• Assess and manage patients presenting with a wide range of conditions in surgery and home visits.
• Practise autonomously within governance framework and Practice direction.
• Make timely referrals to secondary care and community services.
• Support shared decision-making with patients.
• Prescribe safely and cost-effectively.
• Maintain accurate clinical records using the Practice system.
• Participate in investigations of complaints and incidents.
• Engage in Performance Reviews and CPD planning.
