Job summary

Cwmtawe Medical Group in Wales is seeking a Salaried General Practitioner to provide high-quality medical care to registered patients. Responsibilities

• Undertake consultations with patients presenting with acute, chronic and undifferentiated conditions.
• Assess, investigate, diagnose and manage patients appropriately.
• Develop appropriate treatment and management plans in partnership with patients.
• Prescribe medication safely and appropriately in accordance with evidence-based practice, national guidance and practice protocols.
• Review medication for effectiveness, safety and ongoing clinical need.
• Arrange and interpret appropriate investigations.
• Make referrals to secondary care and other health and social care services where clinically appropriate.
• Provide appropriate health promotion, disease prevention and lifestyle advice.
• Provide appropriate management and monitoring of patients with long-term conditions.
• Recognise and respond appropriately to urgent and emergency presentations.
• Undertake home visits where clinically appropriate and in accordance with practice arrangements.
• Provide telephone, digital and face-to-face consultations in accordance with the practice model of care.
• Provide appropriate follow-up and continuity of care.
