An excitingopportunity has arisen for a GP to work within Holgate PCNs Urgent Care Hubsupporting our 3 PCN Practices with a population of 45k patients. We arelooking for an individual who is enthusiastic, highly motivated & who willcontribute to our provision of high quality, patient centred care. We areparticularly looking for a newly qualified GP who is within two years of the date of their certificate of completion oftraining (CCT), issued by the General Medical Council. GPs who are within a few months of completionof training may also be considered.
We are looking for a hardworking, committed GP whose duties will includeassessing, diagnosing, and treating predominantly acute, same daypresentations. Duties will also include working as part of our Urgent Care Hubteam, contributing to the development of our hub & integrating within ourPCN team, working as instructed by our Clinical Director & Senior Manager.
Holgate Primary Care Network (PCN) consists of the following 3 practicesin the Middlesbrough area. Linthorpe Surgery, Borough Road & NunthorpeMedical Group & Village Medical Centre. Two of our practices are wellestablished training practices with excellent GP Support. We have a highlyskilled & varied PCN team consisting of many roles such as Mental HealthPractitioners, Physiotherapists, Care Home Practitioners, Social Prescribers,Safeguarding Lead etc alongside a supportive management and care coordinatorteam.
Our Urgent Care Hub team consists of x2 Advanced Nurse Practitioners, anAdvanced Paediatric Practitioner and Paramedic. This role will involve workingclosely with this team and support will be embedded from our current GP team.
The postholder will:
Manage anddeal with a wide range of presenting health care needs in a primary caresetting.
Work with clinical colleagues to ensure the highest standards of careare provided for all registered and temporary patients.
Undertake a variety of duties such as face to face consultations,telephone consultations, prescribing, dealing with queries, home visits andgeneral duties associated with a GP working in Primary Care.
Contributeto planning and decision-making as part of the wider PCN team.
Review and always adhere topractice protocols and policies.
Attendtraining events organised by the PCN, where appropriate.
