Job summary

Leeds Community Healthcare NHS Trust is recruiting an Occupational Therapist to join a mixed rotation spanning four organisations across Leeds. The role offers opportunities in physical and mental health settings, across community and in-patient environments, with nine-month rotations and strong CPD support.

You will carry a caseload, work autonomously with support from senior therapists, and participate in service improvement projects across the Trust and partners.

Qualifications

• Registered Occupational Therapist with appropriate professional registration.
• Willing to work flexibly, including shifts and weekends.
• Ability to work in various teams and on different sites across Leeds.

Responsibilities

• Manage long-term conditions and falls prevention.
• Urgent response to prevent hospital admissions.
• Facilitate hospital discharge using Home First principles.
• Develop wide and varied skillsets with health coaching and self-management approaches.
