An exciting opportunity has arisen within the Hillingdon Community Rehabilitation Service for a Band 6 Occupational Therapist to join our physical health team.

Community Adults Rehabilitation Service is looking for a confident and experienced occupational therapist who is well motivated to maintain the excellent standard of occupational therapy provision and is passionate about using occupational therapy to enhance treatment outcome and assist clients on their journey of recovery.

The post holder will work closely with the ward therapists and MDT, acute hospital, adult social care, Hillingdon Hospital therapists and rapid response team to ensure seamless transition of care. The post holder will be expected to manage a caseload and assess and treat people on an intermediate care ward.

As an occupational therapist, you will be working in an integrated way and provide a high quality, client-led and goal focused rehabilitation. Continuing Professional Development is actively supported via clinical supervision, peer support, Personal Development Reviews and in-service training.

Main duties of the job

The inpatient rehabilitation post is based at Hawthorn Intermediate Care Unit (HICU) The caseload offers a mix of complex rehabilitation and disability management. You will develop skills in case working and complex case management. Duties include, but are not limited to provide specialised occupational therapy advice in relation to patients 18 years and above with a range of psycho-social, physical and emotional problems working in a rehabilitation setting.

To ensure safe, effective and efficient occupational therapy practice within this field of practice.

To work independently seeking support from the seniors when required.

To accept clinical responsibility and professional autonomy for a designated caseload of patients, from referral to discharge, and to organise this efficiently and effectively in relation to clinical priorities and use of time.

To be professionally and legally responsible and accountable for all aspects of your own work, including the management of patients in your care.

Detailed Job Description And Main Responsibilities

Inpatient rehab - You will be part of a team where your proven ability to make high-level clinical decisions will aid patient recovery. This will include having a direct link to the patient’s medical and therapy team for liaison and support where necessary, and with the patient’s GP or other health care professional as required. You will provide complex assessments and interventions and will make high-impact clinical decisions. You will be working autonomously whilst drawing assistance from colleagues and wider MDT.

Education And Qualifications

Essential criteria

  • Degree / Diploma Occupational Therapy
  • Registration with HCPC as an Occupational Therapist
  • Evidence of relevant CPD

Desirable criteria

  • Membership and/or involvement in special interest group

Previous Experience

Essential criteria

  • Evidence of completing core rotations as a band 5 /student placements and /or experience in a variety of clinical settings including Stroke rehabilitation and management of neurological conditions.
  • Evidence of relevant post graduate courses/ CPD.
  • Evidence of experience and learning demonstrated by professional portfolio.
  • Experience in equipment assessment and provision.
  • Experience of MDT working.
  • Experience of use of OT outcome measures.

Desirable criteria

  • NHS Experience in the community
  • Experience of teaching, supervision and mentoring of support staff or under graduate students.
  • Experience of working at Band 6 level.
  • Experience of audit/research.
  • Awareness and understanding of seamless service across health providers

  • Administration and interpretation of a variety of standardised assessments.
  • Ability to plan, deliver and evaluate OT programmes to address complex and changing health, functional and social well being needs
