What You’ll Do
  • Deliver evidence-based respiratory care within an established team.
  • Assess, monitor, and review patients referred for:
    • Pulmonary Rehabilitation
    • COPD Supported Discharge
    • Home Oxygen Assessments
  • Develop and implement patient‑centred care plans.
  • Assess suitability for LTOT and ensure appropriate provision.
  • Provide specialist care for COPD and ILD patients.
  • Support Community Pulmonary Rehabilitation, including education and exercise sessions.
  • Promote patient self‑management and behaviour change.
  • Communicate complex clinical information clearly to patients, families, and other professionals.
  • Prioritise and manage a varied caseload.
  • Maintain accurate electronic records and support audit activity.
  • Work collaboratively with community nursing and multidisciplinary teams.
  • Adhere to all professional and organisational policies.
