East Suffolk and North Essex NHS Foundation Trust is seeking a Renal Counsellor (NHS Nephrology) in Ipswich to provide psychosocial support to patients with kidney disease, including those undergoing dialysis or pre-transplant, as part of our nephrology team.

You will perform psychosocial assessments, identify barriers to treatment, and work with a multidisciplinary team to develop personalized care plans that aim to improve quality of life for patients with CKD.

Qualifications

• Psychosocial assessments to inform care plans.
• Identify barriers to treatment and coordinate referrals.
• Maintain confidential records of counselling sessions and plans.

Responsabilités

• Provide patient counselling and support through all CKD stages.
• Support patients through dialysis initiation, transplantation readiness, and conservative management.
• Identify and address barriers to treatment adherence and coordinate with MDTs.
