Qualifications

• Registered OT with HCPC.
• Experience in community OT, reablement or hospital discharge.

Responsibilities

• Triage referrals for Occupational Therapy and Independent Living.
• Conduct telephone, virtual and face-to-face assessments where appropriate.
• Identify risk levels and urgency; prioritise referrals.
• Provide advice, information and self-management strategies.
• Recommend equipment, adaptations and assistive technology.
