Job summary

Your World is seeking an experienced Locum IP Pharmacist to join a primary care team in a remote capacity, delivering specialist clinical pharmacy services and medicines optimisation across a patient population in Basingstoke. ASAP start with remote work from home and weekday hours.

The role requires CPPE pathway completion or enrolment, a Master’s in Pharmacy, GPhC registration, and significant post-qualification experience in primary care and other settings.

Qualifications

• Must have completed the CPPE pathway or be currently enrolled on the CPPE pathway.
• Master's degree in Pharmacy and GPhC registration are essential.
• Significant post-qualification experience including primary care and other settings.

Responsibilities

• Independently assess patients and prescribe within clinical competence.
• Lead medicines optimisation initiatives to improve outcomes and cost-effective prescribing.
• Conduct structured medication reviews for complex patients including polypharmacy.
• Manage medicines reconciliation and transfers of care between settings.
• Support chronic disease management through clinical reviews with GPs and nurses.
• Identify and resolve prescribing issues, shortages and interactions.
• Develop prescribing guidelines in line with national guidance and local policies.
• Lead clinical audits and contribute to service improvement.
