Job summary

Liaison Group in the United Kingdom is seeking a Registered Learning Disabilities Assessment Manager to lead complex CHC assessments within our Liaison Care Transformation team. You will review clinical records, assess ongoing support needs and provide evidence-based recommendations to ensure appropriate care and funding.

You will work closely with commissioners, providers and Local Authority representatives to redesign person-centred care packages, develop KPIs, and produce robust clinical

Qualifications

• Learning Disabilities CHC assessments.
• Experience redesigning complex package of care to meet clinical and social care needs.

Responsibilities

• Conduct CHC assessments and collect paperwork.
• Evaluate individual needs based on records, care plans and daily living activities.
• Provide clinical advice and monitor care.
• Review cases to ensure eligibility and appropriate resource levels.
• Determine eligibility and recommendations for health and social care.
• Complete assessment documentation with Local Authority input.
• Ensure Care Plans are updated based on recommendations.
• Draft Recommendations for ICB with clear supporting evidence.
• Support patients and families through the assessment process.
• Provide evidence for all cases where there is a complaint.
• Collaborate with other professionals.
