Job summary

The Methodist Church in Britain is seeking an Independent Prescribing Pharmacist to support anticoagulation DOAC clinics across Waltham Forest, Tower Hamlets, Havering and Newham. Qualifications

• Independent prescriber qualification required.
• At least two years' pharmacist experience in practice.
• Experience with anticoagulation services (DOAC) preferred.

Responsibilities

• Clinically assess and treat patients within a multidisciplinary team using DOAC anticoagulation expertise.
• Take responsibility for care management of patients in anticoagulation services.
• Carry out structured medication reviews to optimise medicines and promote self-care.
• Provide specialist medicines expertise to address public health and social care needs across the network.
• Lead medicines optimisation and antimicrobial stewardship activities.
• Support delivery of the Network DES service specifications and quality outcomes framework.
• Integrate general practice with wider healthcare teams including community and hospital pharmacy.
• Engage in shared care protocols and clinical research with medicines; liaise with specialist pharmacists.
