WF GP FedNet in London is recruiting an IP Clinical Pharmacist to support anticoagulation DOAC services across four boroughs. The role offers remote working from home, with potential travel to clinics as needed, and will operate within a multi-disciplinary primary care network.

The post focuses on medicines optimisation, structured medication reviews, and contributing to antimicrobial stewardship and patient care in NHS-led programmes.

Formación

• Must be an Independent Prescribing Pharmacist.
• Independent prescriber but also with completed ACP.
• Desired start date as soon as possible.

Responsabilidades

• Work as part of a multi-disciplinary team in a patient-facing role to assess and treat patients using anticoagulation knowledge.
• May be required to work in different boroughs.
• Take responsibility for care management of patients for anticoagulation services.
• Provide leadership on medicines optimisation and contribute to quality improvement within the network.
