Practice Plus Group is seeking a Remote Advanced Clinical Practitioner to conduct telephone and digital assessments for urgent care needs. You will work within a multidisciplinary team, making safe decisions and directing patients to appropriate services.

Registration as a nurse with MSc in advanced clinical practice and V300 prescribing qualification are required. Role involves flexible remote working, with shift patterns, and collaboration with GPs and other clinicians to ensure integrated

Formación

• Must have MSc in Advanced Clinical Practice or equivalent.
• Must hold V300 prescribing qualification or equivalent.
• Registered nurse with current registration.
• Experience in out-of-hours, primary care GP surgery or urgent treatment centre, with minimum 6 months post-qualification.

Responsabilidades

• Deliver remote clinical assessments by telephone and digital platforms for patients with urgent and unscheduled care needs.
• Make safe, evidence-based decisions to direct patients to self-care, primary care, urgent treatment centres or emergency services.
• Apply advanced clinical knowledge to assess undifferentiated presentations and manage risk.
• Support clinical governance, contribute to service development and mentor colleagues.
• Work with GPs, paramedics, mental health professionals and other clinicians to integrate care across the region.

This remote Advanced Clinical Practitioner post sits within Practice Plus Group's Clinical Assessment Service, supporting patients across the region. The core of the work is telephone and digital triage, assessing urgent and unscheduled care needs and directing people to the right service. The role is fully remote, working as part of a multidisciplinary team, with shift-based working. Applicants need a nursing registration, an MSc in advanced clinical practice or equivalent, and a V300 prescribing qualification.
