Job summary

Practice Plus Group in Warwickshire is recruiting contracted Advanced Clinical Practitioners for its Out of Hours service across sites in the county. The role involves assessing and treating urgent patients through clinic consultations, home visits and telephone triage, with hours 18:30 to 08:00 on weekdays and 24/7 at weekends.

You will be registered nurse or paramedic with MSc in Advanced Clinical Practice and V300 prescribing.

Responsibilities

• Assess, diagnose and treat patients with urgent healthcare needs.
• Carry out face-to-face consultations from the clinical base.

• Manage telephone triage, giving clinical advice and directing patients to care pathways.
• Assist with telephone triage calls when face-to-face bookings are low.
• Work autonomously and collaborate with a multidisciplinary team.
• Keep accurate clinical records and follow NHS guidelines.

Job description

Practice Plus Group is recruiting contracted Advanced Clinical Practitioners for its Warwickshire Out of Hours service, which operates from sites across the county. The work is assessing and treating urgent, unscheduled patients through clinic consultations, home visits and telephone triage. The service runs 18:30 to 08:00 on weekdays and round the clock at weekends, taking referrals from 111, paramedics, district nurses, emergency departments and minor injury units. Applicants need to be registered nurses or paramedics with an MSc or equivalent and a V300 prescribing qualification.

What the job involves

• Assess, diagnose and treat patients with urgent healthcare needs
• Carry out face-to-face consultations from the clinical base
• Make home visits to patients who need assessment or treatment in their own residence

• Assist with telephone triage calls when face-to-face bookings are low

Experience
