Forward Trust in the United Kingdom is seeking a compassionate RECONNECT Practitioner to support individuals before and after release, helping them access health, wellbeing and community services to reintegrate.

You will deliver personalised support for up to six months after release, complete holistic needs assessments, and develop tailored plans while building strong links with prisons, healthcare providers and local agencies.

Qualifikationen

• Experience supporting vulnerable people or working in health, social care, criminal justice or a related field is desirable.
• A caring, organised and proactive individual with excellent communication skills.
• The ability to manage a varied caseload and work independently while contributing to a supportive team.

Aufgaben

• Deliver personalised support to people preparing for release and for up to six months afterwards.
• Complete holistic needs assessments and develop tailored support plans.
• Build strong relationships with prisons, healthcare providers and community organisations.
• Connect service users with GPs, mental health services, wellbeing support and local agencies.
• Manage referrals, maintain accurate records and work collaboratively to achieve positive outcomes.
