Job summary

The Forward Trust is recruiting a RECONNECT Support Worker to assist individuals leaving prison in Bedfordshire and Milton Keynes. This permanent role focuses on health, wellbeing and community reintegration within NHS:commissioned services.

You will assess needs, develop personalised care plans, and support clients through referral access to GPs and community services with ongoing post-release contact for up to six months.

Qualifications

• Lived experience of criminal justice system or related health challenges.
• Non-judgemental and compassionate approach.
• Ability to build trust with people from diverse backgrounds.
• Excellent communication and listening skills.
• Strong organisational and administrative skills.
• Ability to complete clear case notes and support plans.

Responsibilities

• Manage referrals and carry out health and wellbeing assessments.
• Develop individualised care and support plans.
• Build positive relationships and provide ongoing support.
• Manage a busy and varied caseload.
• Help people prepare for release and reintegrate into the community.
• Assist service users to access healthcare and community services.
• Provide guidance and signposting based on individual needs.
• Maintain contact for up to six months after release.
• Ensure people are connected with services before leaving prison.
• Collaborate with colleagues to share best practices and manage risks.
• Contribute to delivering a safe, person-centred service.
