Job summary

Rhondda Cynon Taf Council is seeking a Rehabilitation Worker to join the Rehabilitation Service on a permanent, part-time basis (16 hours per week). The role supports people to regain independence and daily living skills at home, with a focus on person-centred care and collaboration with health and social care partners.

The position is based at Ty Elai, Williamstown, with a salary at Grade 5 and a pension scheme.

Responsibilities

• Provide practical support, reassurance and encouragement to individuals following illness, hospital admission or a period of reduced independence.
• Help people maintain, regain or learn everyday living skills, enabling them to reach their full potential and remain safely at home wherever possible.
• Support individuals to complete an agreed programme of rehabilitation support.
• Promote independence, confidence and wellbeing through practical, person-centred support.
• Work closely with colleagues from health, social care and other partner agencies.
• Communicate effectively with individuals, families, carers and professionals to help achieve positive outcomes.
• Contribute to team meetings, supervision sessions and regular observations of work practice.

About the Role

Rhondda Cynon Taf Council is pleased to offer an opportunity to join the Rehabilitation Service as a Rehabilitation Worker. We are looking to appoint a motivated, compassionate and enthusiastic individual to support people across Rhondda Cynon Taf to regain confidence, independence and essential daily living skills.

The Rehabilitation Service is a joint health and social care initiative, bringing together Occupational Therapists, Physiotherapists, an Occupational Therapy Assistant, Assistant Therapy Practitioners, a Team Lead and a highly trained team of social care staff. Together, the service provides short-term support, usually for up to six weeks, to help individuals recover from ill health, prevent deterioration where possible and reduce the need for longer-term support.

What You Will Do

As a Rehabilitation Worker, you will provide practical support, reassurance and encouragement to individuals following illness, hospital admission or a period of reduced independence.
