Job summary

Active Care Group is seeking a Rehabilitation Support Worker at Rowland House, Thames Ditton, Surrey. The role is a full-time 42-hour contract on a rota that includes every other weekend, with a salary of £31,098.09 per annum FTE pro rata.

You will support residents across mental wellbeing, independent living, health needs and community integration, helping them lead safer, happier lives while developing daily living and budgeting skills.

Qualifications

• Enhanced DBS required.
• Flexible, professional and positive approach to multi-site working.
• Proven reliability in terms of full attendance and timekeeping.
• Ability to work as part of a team and as an individual.

Responsibilities

• Support residents to achieve outcomes in mental wellbeing and independent living.
• Assist with medication management and health needs as directed.
• Encourage community integration, education, volunteering and employment opportunities.
• Help build relationships, financial management and budgeting skills.
• Support meaningful occupation, daily activities and risk management.
• Assist with personal care as required.
