Herefordshire and Worcestershire Health and Care NHS Trust is recruiting a Band 3 Healthcare Support Worker to join the Pershore Community Hospital ward. The role supports an MDT in rehabilitation goals and patient care, with a focus on delivering safe, effective and compassionate care close to home.

The post involves assisting with admissions, daily living activities, meals, and basic clinical tasks under supervision, contributing to shorter lengths of stay and high-quality patient experience.

Responsabilidades

• Assist with admitting patients to the ward and the care of their relatives
• Carry out delegated nursing care as assessed by senior staff
• Perform weekly patient assessments including weighing and mattress checks
• Support patients with activities of daily living and rehabilitation
• Assist with meals, personal care, and mobilising patients using mobility equipment
• Record paperwork such as food diaries and fluid balance charts
• Liaise with nursing and physiotherapy teams for observations, catheters, samples and health checks
• Ensure stores are restocked and policies/procedures are followed
