Job summary

Cornerstone Healthcare Group are seeking experienced Healthcare Assistants to join The Burren in Bristol. This role focuses on rehabilitation, complex dementia care, and mental health support, working with a multidisciplinary team to deliver person-centred care.

You’ll work 38.5 hours per week on 12-hour shifts (days and nights) with weekend rotation. Note: sponsorship is not available and a £500 Welcome Bonus applies to successful applicants.

Qualifications

• At least 12 months' experience in rehabilitation, recovery, or case management settings.
• Experience with multidisciplinary teams (occupational therapists, physio, GP, social workers).
• Knowledge of person-centred and recovery-focused approaches.
• Understanding of rehabilitation pathways.
• Strong observation, documentation, and reporting of progress.
• Calm under pressure with emotional resilience.
• Caring, compassionate, and patient-centred approach.
• Excellent communication and team collaboration.

Responsibilities

• Support residents with recovery and rehabilitation goals.
• Work with nurses, therapists and the MDT to implement care plans.
• Monitor and report changes in residents' wellbeing accurately.
• Provide high-quality personal care and daily living assistance.
• Build trusting relationships considering individual needs and preferences.
• Maintain records and contribute to care planning processes.
• Promote dignity, privacy and respect at all times.
