Job summary

The Optalis Short-Term Support and Rehabilitation Team (STS&R) provides reablement care and support to people across the Royal Borough of Maidenhead and Windsor.

Reablement is a short-term, home-based support service that helps individuals regain skills, abilities, and confidence to manage daily tasks and live as independently as possible. It is provided after illness, injury, or hospital stay, focusing on supporting people to do things for themselves again, rather than doing things for them. With personalized recovery goals and plans created with our team of Occupational and Physiotherapists, reablement aims to improve independence through practical skills and activities.

In simple terms, reablement helps individuals return to routines like washing, dressing, cooking, or moving around, promoting independence.

Main duties of the job

We are recruiting Rehabilitation Care Assistants to expand our team and support more customers in their homes.

As a Rehabilitation Care Assistant, you will support customers in their homes to rebuild confidence and regain independence. Working with person-centered care plans developed by our Occupational Therapists and Physiotherapists, you will motivate and engage customers towards their recovery goals. You will also assist with personal care, mobility, medication administration, and some domestic tasks.

This role is based out of Maidenhead Town Hall but involves visiting customers across the borough. You must be able to drive and have your own vehicle.

Full-time shifts typically involve 4-5 days a week, covering AM (0700-1500) and PM (1400-2200) shifts, including alternate weekends.

Customer support is limited to a maximum of six weeks, so caseloads vary weekly, making each week unique.

Job description

Responsibilities include:

  • Deliver safe, person-centered support at customers' homes, including personal care, meal prep, medications, and rehabilitation activities.
  • Provide detailed feedback to the therapy team and coordinators to update rehabilitation plans as clients progress, using provided equipment.
  • Report any deterioration or change in condition for medical intervention, liaising with healthcare professionals as needed.
  • Use initiative and decision-making skills to ensure safety and well-being in the community.
  • Participate fully in team meetings, supervision, and appraisals.
  • Work with families and carers to explain and support the rehabilitation process.

Qualifications (Essential):

  • Ability to work independently

Experience (Desirable):
