Job summary

Optalis is recruiting a Rehabilitation Care Assistant for the Short-Term Support & Rehabilitation (STS&R) team. You will work in a busy, multi-disciplinary environment with Occupational Therapists and Physiotherapists to deliver personalised 1:1 care and support in clients' homes across the Royal Borough of Windsor & Maidenhead.

Job description

The Short-Term Support & Rehabilitation (STS&R) team is a reablement service dedicated to helping people get back on their feet. When someone has an illness, injury, or a hospital stay, we step in to help them regain the skills and confidence they need to live independently again.

Along with delivering traditional care and support, we also provide reablement we don't just "do things" for our customers; we empower them to do things for themselves, minimising the need for long-term support.

Your Role:

As a Rehabilitation Care Assistant, you are part of a high-level, multi-disciplinary team. Youll work alongside Occupational Therapists and Physiotherapists, making this an incredible opportunity to develop clinical knowledge and explore different career paths within health and social care.

Main duties of the job

What you'll be doing:

• Individualised Care: Assisting with personal care, medication, and physical wellbeing based on specific 1:1 requirements.
• Skill Building: Supporting customers with daily tasks while teaching them techniques to do them independently.
• Collaboration: Working with therapists to follow specific rehabilitation plans.
• Community Impact: Visiting customers in their own homes across the Royal Borough of Windsor & Maidenhead.
• No Night Shifts: Working a mix of morning and afternoon shifts on a rota basis -no nights required.

About You:

Whether you've worked in Domiciliary Care, as a Healthcare Assistant, or are looking for a total career change, we want to hear from you. We provide full, in-house training to get you started.

This is a community-based role and you will be expected to travel across the Royal Borough visiting different people - you must have a full UK driving licence and access to your own vehicle.

Unlike many community roles, you are paid for your entire shift, not just the time spent with customers. We also pay 55p per mile for your travel.

Job responsibilities

The short-term support team is a well-established and valued community based rehabilitation team,providing care and support to people at home.

The service is multidisciplinary, and you will be working alongside occupational therapists,physiotherapists and team coordinators to provide person centred support, helping to achieve agreedgoals to maximise independence.

The service is supportive of both staff and service users and exhibits a culture of transparency andintegrity.

The role of the rehabilitation assistant is integral to the service and vital in enabling the service to supportthose that wish to remain at home for end of life care, prevent hospital admission and enable residentsto leave hospital earlier to continue rehabilitation and recovery at home. The role is supported by a teamof therapists guiding and educating in the delivery of physiotherapy and occupation therapy programmes.

The rehabilitation assistants are given enhanced training to enable the assessment for and delivery ofsmall pieces of equipment and aids to enable independence.

• Must hold entitlement to work - either as a British national or a valid work visa with at least 12 months remaining.
