Job summary

Bandrum Nursing Home is seeking a Rehabilitation Assistant Coordinator to support a defined group of residents to work towards individual rehabilitation and re-enablement goals. You will help residents build and maintain abilities through personalised programmes and meaningful activity while guiding the care team on how to support rehabilitation in everyday care.

This role suits someone who enjoys working closely with people, stays positive when progress is slow, and takes a proactive approach

Qualifications

• Experience in a rehabilitation, re-enablement or therapy support role.
• Interest in rehabilitation, reablement and promoting independence.
• Ability to communicate with residents, families and professionals.
• Confident supporting residents with mobility, exercise and daily activities.
• Maintain accurate records and documentation.
• Understand safe moving and handling practices.
• Willing to undertake training in rehabilitation and postural management.

Responsibilities

• Support residents to maintain and improve independence through personalised rehabilitation and re-enablement programmes.
• Identify meaningful goals with residents and contribute to plans to achieve them.
• Support individual and group activities for physical, emotional and functional wellbeing.
• Assist residents with personal care while encouraging independence.
• Deliver mobility and exercise programmes with the multidisciplinary team.
• Carry out equipment checks and audits (monthly sling and hoist).
• Develop links with external services to broaden opportunities for residents.
• Keep knowledge up to date and contribute to service development.

At Bandrum Nursing Home, rehabilitation is about supporting residents to live as independently, confidently and actively as possible.

We are looking for a Rehabilitation Assistant Coordinator to work closely with residents, colleagues and the wider team to support individual rehabilitation and re-enablement goals.

You will help residents build and maintain their abilities through personalised programmes, meaningful activity and practical support while also helping colleagues understand how they can support rehabilitation throughout everyday care.

THE ROLE

As Rehabilitation Assistant Coordinator, you will support a defined group of residents to work towards individual goals through planned rehabilitation, re-enablement and meaningful engagement.

This will include:

• Supporting residents to maintain and improve their independence through personalised rehabilitation and re-enablement programmes
• Working with residents to identify meaningful goals and contributing to plans designed to help achieve them
• Supporting individual and group activities that promote physical, emotional and functional wellbeing
• Assisting residents with personal care in a way that encourages independence and maintains existing abilities
• Delivering agreed mobility and exercise programmes in conjunction with the wider multidisciplinary team
• Working with postural management and external healthcare professionals to support individual rehabilitation programmes
• Preparing schedules and maintaining accurate records to evidence completion and progress
• Supporting and educating care staff to deliver agreed exercise and postural management programmes safely and consistently
• Providing manual handling guidance and training to the care and support team
• Carrying out relevant equipment checks and audits, including monthly sling and hoist audits
• Developing links with external services and resources to broaden the opportunities available to residents
• Keeping your knowledge of rehabilitation and reablement up to date and contributing to the ongoing development of the service
• Working in accordance with Bandrum's policies, procedures and relevant professional and legal requirements

ESSENTIAL REQUIREMENTS

You will:

• Have previous experience in a rehabilitation, re-enablement or therapy support role
• Have a genuine interest in rehabilitation, reablement and promoting independence
• Be able to communicate effectively with residents, families, colleagues and other professionals
• Be confident supporting residents with mobility, exercise and everyday activities
• Be able to maintain accurate records and documentation
• Understand the importance of safe moving and handling practice
• Be willing to undertake training and continue developing your knowledge of rehabilitation and postural management

DESIRABLE REQUIREMENTS

It would be helpful if you already have:

• Previous experience within a care, healthcare, charity or regulated environment

Hours

Full-time, permanent position. Please note - weekend work will be required.

£13.45 per hour
