This is an exciting opportunity to be part of a new, community-based rehabilitation service, supporting people with complex mental health needs to live independently, develop their recovery and maintain stability within the community.

You will provide direct, person-centred support to individuals, working alongside a multidisciplinary team to help people build skills, confidence and social networks.

The service focuses on helping people to:

• Maintain their independence
• Develop daily living skills
• Build confidence and social inclusion
• Sustain living in their own home or supported accommodation

The team works using an assertive outreach model, delivering care in people's homes and community settings.

As a Mental Health Support Worker, you will:

• Work directly with service users to support recovery and wellbeing
