We Are Hiring

• Department: Case Management
• Schedule: PRN any shift. This is mainly for PTO, call-ins, and staffing vacancies.
• Hospital: St. Vincent's East and St. Clair
• Location: Birmingham, AL

Case management experience is strongly preferred. This person could float between East and St. Clair for coverage. Birmingham is an option as well if there are no shifts available at the other campuses.

What You Will Do

Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.

• Create a plan for care across the continuum, integrating patient/family preferences and values.
• Monitor patient care through assessments, evaluations, and/or patient records.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialogue with supervisors and other health providers to ensure effective implementation of the health plan.
• Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

What You Will Need

Licensure / Certification / Registration:

• Licensed Associate Professional Registered Nurse credentialed from the Alabama Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR required professional licensure at the time of hire.
